Rosaline (69 years old)

Rosaline, 69 years old, consults the urogynaecologist because of persisting urgency, urgency urinary incontinence and frequency. She already tried different formulations of antimuscarinic agents for the past 2.5 years. She experienced some improvement with the last formulation (solifenacin 5 mg).

 

Bladder diary: Frequency: day 9x, night 1x. Urgency episodes: 2-4x/day. Urgency urinary incontinence episodes: 1-2x/day.
Ultrasound: Post void residual: 20 ml.
Uroflowmetry: Voided volume: 180 ml. Qmax: 33 ml/s.

What is the most appropriate treatment choice for this patient?
