Manuel, 74 years old.

Consider this patient’s medical history:

• 5 years ago: radical prostatectomy for T2N0M0 PCa (ISUP grade group 3 (Gleason 4+3))
• 2 years ago: early prostate bed salvage RT without ADT for BCR (PSA: 0.22 ng/ml)

Follow-up information today:

• PSA: 1.2 ng/ml
• PSA-DT: 7 months
• Asymptomatic, no relevant comorbidities
• ECOG PS: 1
• PSMA-PET/CT: uptake in right acetabulum and L1 spinous process with no morphologic lesion on the CT
