George (67 years old)

George, a 67-year-old patient with known type 2 diabetes, presents to your office with a 4-day history of watery diarrhoea 5-6 times per day. Apart from weakness, there are no other symptoms. He tested positive for COVID-19. On initial presentation, he was not febrile, tachypnoeic nor tachycardiac. His blood pressure was 118/70 mmHg and he seemed slightly dehydrated. There was mild generalised abdominal tenderness, but no rigidity or rebound. The rest of the physical examination was unremarkable. He takes metformin and saxagliptin (dipeptidyl peptidase 4, DPP4, inhibitor) and he reports fasting glucose levels between 4-8 mmol/l (70-140 mg/dl).
