Brunhilde is 89 years old and lives in rural Bavaria, a 2-hour car ride away from the next city. Unfortunately, she fell during a walk in the woods and hurt her leg badly. She was transferred to the local hospital, where an X-ray revealed a fracture of the right femur due to a suspicious lesion.

- Medical history:
- Diabetes mellitus type 2
- Ulcerative colitis
- Long QT syndrome
- ECOG PS: 2
- No family history of breast cancer
- Biopsy of the bone lesion: breast cancer, ER 100%, PgR 100%, HER2 IHC 0, G1
- Imaging:
- 2.5 cm tumour in the right breast, 1 suspicious axillary lymph node
- 1 lesion suspicious for metastasis in right femur, no further metastases
- Germline BRCA-status: wild type
- PIK3CA-status in the tumour: wild type
