Angela (64 years old)

Angela is a 64-year-old woman who underwent kidney transplantation 4 years ago. Immunosuppression is controlled with tacrolimus and aprednisolone. She is known to suffer from hypertension and significant coronary heart disease. Angela presents otherwise no health problems nor activities of daily living (ADL) limitations.

Her husband calls you as she could not get out of bed this morning, felt dizzy and had a “clouded consciousness”. Her morning blood pressure was 94/76 mmHg and her husband therefore simply paused the antihypertensive medication, knowing the symptoms from her time undergoing haemodialysis.

She now still feels weak, tired and dizzy and her measured temperature is 37.8°C. Her blood pressure did not rise above 100 mmHg systolic. According to the husband, consciousness was back to normal after her usual morning tea.

What is the best way to verify this patient’s status?
